“Home infusion is historically an old industry,” she explains. “It was founded on mostly acute-type therapy. What’s happened is the drug pipeline has shifted in a way that there are far more infusible specialty drugs on the market to treat diseases we haven’t historically had therapies for.”

They really care about what happens when a patient goes home. In the private world, a company might care that a patient was readmitted, but it doesn’t affect their bottom line.”

“Business continuity becomes a real question,” says Stokes. “One of the things I’m a strong proponent of is building dual clean rooms: two clean rooms, separate ductwork, separate HVAC, full separate mechanical. You can operate them at normal capacity day-to-day. But if you have an HVAC issue or a regulatory closure, you have the flexibility to move to the other room and keep serving your patients.”

 

Health system clean rooms have historically been evaluated as immediate-use environments, where medication is compounded and administered the same day. But home infusion pharmacies dispense medications for up to seven days, creating meaningfully higher contamination risk. The FDA is increasingly evaluating these facilities not as 503A spaces, but closer to 503B standards.

Companies that engage in longer beyond-use dating tend to have more thorough quality programs, more rigorous garbing and cleaning procedures, and more intensive environmental monitoring, standards that health systems haven’t historically been held to. As they expand into home infusion, that gap becomes a real and addressable risk.
